Download and complete the Medical Release and Waiver Form. Be sure to obtain your AAU membership. Upload completed form and your membership proof in the application below.
2. Pay $25 Registration Fee
After submitting the application above, please pay your non-refundable tryout fee of $25 prior to tryouts. Venmo, Cash, or Check accepted at the door. Make sure to include athlete’s name in payment.
